BUCCAL FAT VS MASSETER BOTOX FOR FACE SLIMMING IN SEOUL

Two very different routes to a slimmer face in Seoul: one is permanent surgery, one is a 20-minute jab. Here is how to choose.

Face slimming is one of the top three requests foreigners bring to Seoul clinics, and it is where the biggest decision usually happens in the first ten minutes of consultation. On one side you have buccal fat removal, a permanent surgical procedure that scoops out a pad of fat from inside your cheek. On the other you have masseter botox, a quick injection into the jaw muscle that shrinks it over four to six weeks. They target different anatomy, cost wildly different amounts, and lock you into different futures. Pick wrong and you either overpay for something that will not solve your issue, or you cut something you cannot put back.

THE TWO PROCEDURES SLIM COMPLETELY DIFFERENT ZONES

Buccal fat removal targets the middle third of your face, the area right below the cheekbone. The buccal fat pad sits deep inside the cheek and gives some faces a rounded, youthful fullness. A surgeon makes a small incision inside the mouth, removes a portion of the pad, and closes it up. There are no external scars. The change is permanent because fat pads do not regrow. Recovery in Seoul clinics runs three to seven days of visible swelling, with the final result settling in around three to six months.

Masseter botox works on a different structure entirely. The masseter is the thick chewing muscle at the back corner of your jaw, and in people who grind their teeth or have naturally strong masseters it can bulge outward and create a square, wide lower face. Botox weakens the muscle, and over four to six weeks it atrophies from disuse and shrinks. The result is a softer, more V-shaped jawline. The effect lasts four to six months, then you retreat. Nothing is removed, no incisions, back to normal activity same day. If you want the fuller Korean botox brand context before committing, that comparison is worth reading first.

HOW TO TELL WHICH ONE YOU ACTUALLY NEED

The mirror test is fast. Clench your jaw hard and feel the back corner of your jawbone with your fingers. If a muscle bulges out and gets rock hard, that is your masseter, and it is a major contributor to the width of your face. Masseter botox will help. Now unclench and press gently inward on your cheek just below the cheekbone. If your face feels soft and full there even at your normal weight, and your face looks rounder than the rest of your body suggests it should, that is likely buccal fat. Seoul consultants call this the difference between muscle-driven width, which sits low and back, and fat-driven fullness, which sits high and central.

Plenty of foreigners walk in convinced they need buccal fat removal when the real issue is a strong masseter. The opposite happens too. A quick under-cheekbone push and jaw clench during your first consultation should sort it. Most Gangnam and Apgujeong clinics offer a free 15-minute assessment where a doctor palpates both zones and photographs your face at rest and in animation. Take that. Do not book surgery on the first visit if the doctor is not clear which mechanism is driving your concern.

SEOUL PRICING IN 2026: THE GAP IS BIGGER THAN YOU THINK

Buccal fat removal in Seoul in 2026 typically runs ₩1,500,000 to ₩3,500,000, roughly 90 to 60. Apgujeong and Cheongdam clinics sit at the top of that band. Sinnonhyeon and Sinsa run 20 to 30 percent lower for the same board-certified plastic surgeon quality. Add ₩150,000 to ₩300,000 for a follow-up ultrasound or de-swelling laser sessions if your clinic offers them. VAT is usually included for foreign patients, but confirm before you sit in the chair. If you want a sharper read on how VAT and package pricing hide costs, the hidden costs guide walks through the common traps.

Masseter botox is a fraction of that. In Seoul in 2026 the price per session for 50 to 100 units of a Korean brand like Botulax, Nabota, or Meditoxin runs ₩150,000 to ₩350,000, about 90 to 65. Allergan Botox runs 50 to 80 percent higher for the same unit count. Two sessions per year to maintain the effect is standard, so annual spend on masseter botox lands around 30 to 25. Over five years a maintenance program costs less than a single buccal fat surgery. If cost is the primary driver, masseter botox is not close.

THE RISK PROFILES ARE NOT COMPARABLE

Buccal fat removal is real surgery. Complications are uncommon in experienced Seoul hands, but they exist: asymmetry, over-resection causing gaunt cheeks in your forties and fifties as your face naturally loses volume, salivary duct injury, and infection. The gaunt-cheek risk is the one seasoned consultants raise most often. A twenty-something with rounded cheeks may love the result at 28 and hate it at 45, because you cannot add the pad back. Some Seoul surgeons now refuse buccal fat removal on patients under 25 for this reason, or cap the amount removed at 40 percent.

Masseter botox risks are limited and reversible. The main downsides are a temporary hollowing under the cheekbone if the injection diffuses upward, difficulty chewing tough foods for the first two weeks, and, rarely, an asymmetric smile if placement is off. Everything wears off in three to six months. That reversibility is why Seoul clinics treat masseter botox as a lifestyle maintenance treatment and buccal fat as a considered surgical decision. If bruxism or TMJ tension is part of your picture, the bruxism and TMJ botox guide covers the medical side of the same injection.

WHAT A REALISTIC SEOUL TRIP LOOKS LIKE FOR EACH

For masseter botox the trip is trivial. Book a consultation and injection on day one at any reputable Gangnam or Sinnonhyeon clinic. Injection takes ten to fifteen minutes. You can eat, walk, shop the same afternoon. You will not see the slimming effect until week four to six, so this is a treatment you fly home from and then admire in your bathroom mirror a month later. Combine it with lighter treatments on the same trip, for example skin boosters or a HydraFacial, without conflict.

Buccal fat removal is a proper medical trip. Plan seven to ten days in Seoul minimum. Day one is consultation and pre-op photography. Surgery day two or three, under local anesthesia with sedation. Days four through seven you will be visibly swollen, on a soft food diet, sleeping elevated. Most Seoul plastic surgery clinics book a mandatory day-seven follow-up before you fly out, and clearance to fly usually comes at day five to seven. If you want to stack this with any other injectable treatment, do the injectables in a second trip. Combining surgery day with fresh filler or thread is asking for asymmetric swelling that muddies both results.

WHICH SHOULD YOU ACTUALLY BOOK?

Book masseter botox first, in almost every case, if you have not tried it. It is cheap enough to treat as a diagnostic. If four to six weeks after your injection your jaw looks noticeably softer and you are happy, you never needed surgery. If the masseter shrinks but your face still looks round in the mid-cheek, then buccal fat removal on a follow-up trip becomes a much more precise decision, because you have already isolated what the muscle contribution was. This sequencing is what experienced Seoul consultants recommend to foreign patients who are trying to decide between the two. Rushing straight into surgery without ruling out the masseter is the single most common reason people end up regretting an irreversible decision.
