GUMMY SMILE BOTOX IN SEOUL 2026: FIX WITHOUT SURGERY

Seoul clinics treat a gummy smile with 2 to 4 units of Botox into the LLSAN muscle for about ₩60,000, and results last three to four months.

A gummy smile is one of the fastest concerns to fix in Seoul, and one of the cheapest. Two to four units of Botox into the right muscle drops the upper lip by two or three millimetres, and the whole appointment fits inside a lunch break. Seoul clinics have been doing this injection thousands of times a year since the mid-2010s, which is why the technique is refined, the pricing is honest, and the recovery is close to nothing.

WHAT ACTUALLY CAUSES A GUMMY SMILE?

The muscle at fault is the levator labii superioris alaeque nasi, usually shortened to LLSAN. It runs from the side of the nose down to the upper lip, and when it contracts too hard it hoists the lip up past the tooth line and exposes three, four, sometimes six millimetres of gum. Most people who show gum when they smile are showing between two and four millimetres, which reads as high smile line rather than a full gummy smile. Anything above three millimetres is where clinics start recommending correction, and Seoul dermatologists tend to be conservative about it. If the cause is a short upper lip or a hyperactive LLSAN muscle, Botox is the answer. If the cause is vertical maxillary excess, meaning the upper jaw itself sits too low, no injection will fix it, and a proper Seoul clinic will tell you that on the first consultation instead of selling you a treatment that will not work.

There is a second sub-type worth knowing. Some gummy smiles are caused by short clinical crowns, where the teeth themselves are partly covered by gum tissue. That is a dental problem, not a muscle one, and the fix is a gingivectomy at a Seoul dental clinic, not Botox at an aesthetic one. A good injector will look at your teeth-to-gum ratio at rest before agreeing to inject. If the injector skips that check and goes straight to marking your face, walk out.

HOW THE SEOUL BOTOX PROTOCOL WORKS

The injection itself takes about 90 seconds. The doctor asks you to smile at full effort, marks a point on either side of the base of the nose where the LLSAN sits, and delivers two units per side with a 32-gauge needle. Total dose is four units in most cases, sometimes six for a stronger muscle, almost never more than eight. Onset is not immediate. The lip starts to drop around day three and finishes settling around day ten. Peak effect sits from week two to about week twelve, and most people notice the muscle waking up again around month three. This is shorter than the four-to-six months you get from masseter or forehead Botox because the LLSAN is a smaller muscle metabolising a smaller dose. WHAT DOES A GUMMY SMILE FIX COST IN SEOUL?

This is one of the cheapest injectables on the Seoul menu. A LLSAN dose using Korean-made Botulax runs about ₩50,000 to ₩80,000 for the whole treatment, which is around USD 37 to USD 60 at 2026 exchange. Nabota is a small step up, usually ₩70,000 to ₩100,000. Innotox, the liquid formulation preferred by some injectors for micro-doses because it does not need reconstitution, sits at ₩90,000 to ₩130,000. Allergan Botox for this indication is rare in Seoul because the volume is so low that clinics do not want to open a fresh vial for four units, but where offered it runs ₩150,000 to ₩200,000.

For context, the same procedure in Bangkok runs about USD 90 to 150, in Sydney about AUD 250 to 400, and in Los Angeles USD 300 to 500. Seoul is the cheapest credible market in the world for this specific injection, and the pricing is stable across the tier of clinic you pick because the dose is so small. HOW LONG DOES GUMMY SMILE BOTOX LAST?

Three to four months on the first round, four to five months by the third round, sometimes longer once the muscle is trained. This is the payoff for staying on schedule. LLSAN is a small, superficial muscle, and if you keep it partly relaxed for a full year, it stops firing as hard even during the off-weeks between injections. Most Seoul dermatologists tell foreign patients to plan three visits in the first year if they want to lock in the effect. There is one important trade-off. Over-injecting the LLSAN, or hitting it slightly off target, drops the lip too far and flattens the upper lip when you talk. This produces a stiff, slightly frozen smile and can make the philtrum look longer than it is. Seoul injectors correct this by starting low, usually two units per side, and adding one extra unit at the two-week check if the drop is not enough. Do not accept a first-visit dose above six total units for a standard LLSAN correction. If the doctor pushes higher on the first visit, that is the moment to ask why, and if the answer is vague, book somewhere else.

WHERE TO BOOK IN SEOUL, AND WHAT TO ASK

Apgujeong and Cheongdam are the two districts where most foreign patients get this done, mainly because the English-support clinics cluster there and the surrounding blocks are easy to walk around while you wait for the numbing cream to kick in. Sinnonhyeon, one subway stop north of Gangnam Station, is the price-conscious alternative. It has the highest concentration of injectable clinics per square kilometre in Seoul, and LLSAN doses run at the low end of the range because clinics compete on volume. Myeongdong is the tourist-facing option and works fine for a one-off, though pricing tends to sit ₩10,000 to ₩20,000 higher for the same brand.

At the consultation, ask three questions before you agree to the injection. First, how many millimetres of gum do I show at full smile, and how many are you aiming to drop me to? A confident injector will answer in specific numbers, not adjectives. Second, which brand are you using and what is the total unit count? Get this in writing on the estimate. Third, when is the two-week check-in, and is it included in the price? Most Seoul clinics include one free touch-up at two weeks if the effect is under-dosed, which is the correct way to run this protocol.
