VAT AND TAX REFUND FOR SEOUL PLASTIC SURGERY 2026: WHAT FOREIGNERS CAN ACTUALLY CLAIM

Cosmetic surgery in Korea carries 10% VAT, and foreign patients can reclaim most of it. Here's which procedures qualify, which clinics process the refund cleanly, and what to carry to Incheon on your way home.

Korea charges 10% VAT on cosmetic plastic surgery, and most foreign patients can claim it back. The rule has been on the books since 2016 but the process still trips up first-time medical tourists because the paperwork lives at the clinic, not the airport. If you spend ₩15,000,000 on rhinoplasty with revision, that is up to ₩1,500,000 sitting on the table if you leave Incheon without the right slip. This guide walks through what qualifies, which clinics make the refund painless, and what to hand the tax desk at the terminal.

WHY COSMETIC SURGERY IS TAXED IN THE FIRST PLACE

Medical treatment in Korea is normally VAT-exempt. Cosmetic procedures were carved out in 2011 and now carry a 10% VAT line that Korean residents cannot reclaim. Reconstructive work after accident, burn, or cancer stays exempt, and functional procedures like septoplasty for breathing or ptosis correction for lid droop can qualify as exempt if the surgeon documents the medical indication. Pure aesthetic rhinoplasty, breast augmentation, facelift, two-jaw contouring, blepharoplasty, and liposuction all carry the 10%. See the clinic-by-clinic cost context in our rhinoplasty cost guide if you want to understand how VAT nests inside the quoted price.

THE MEDICAL VAT REFUND SCHEME FOR FOREIGN TOURISTS

The Korean government opened a medical VAT refund route for inbound patients in April 2016. It runs through two licensed operators, Global Tax Free and Global Blue Korea Tax Free, and it covers cosmetic surgery performed at clinics registered under the scheme. You must hold a foreign passport, have been in Korea for less than six months at the time of treatment, and leave the country within three months of the procedure. Refunds land at roughly 7 to 9 percent of the pre-tax base after operator fees, which means on a ₩20,000,000 surgery you typically see ₩1,400,000 to ₩1,600,000 back.

WHICH PROCEDURES ACTUALLY QUALIFY

  • 01Rhinoplasty, including primary, revision, and tip plasty
  • 02Blepharoplasty: incisional and non-incisional double eyelid, canthoplasty, epicanthoplasty
  • 03Facial contouring: V-line, zygoma reduction, genioplasty, buccal fat removal
  • 04Facelift: SMAS, deep plane, mini-lift, neck lift, fat grafting
  • 05Breast augmentation with implants, fat transfer to breast, and revision
  • 06Body contouring: liposuction, 360 lipo, tummy tuck, arm and thigh lift
  • 07Orthognathic two-jaw surgery performed for aesthetic indication
  • 08Hair transplant via FUE, DHI, and robotic methods

Procedures outside the scheme include dental implants billed on a separate invoice, oriental medicine, and anything coded as exempt reconstructive treatment where the hospital already removed the VAT line. Non-surgical treatments like botox, fillers, lasers, and skin boosters sit under the standard tourist VAT refund rather than the medical scheme, with a different form at the airport.

CLINICS THAT PROCESS THE REFUND IN-HOUSE

Not every Seoul clinic is registered. The ones that handle it most smoothly are the government-designated foreign patient hospitals, which carry the KFBA or MOHW accreditation on their licence wall. Girin Plastic Surgery in Seocho and Grand Plastic Surgery in Apgujeong are both government-designated for international patients and run a dedicated tax desk alongside the international lounge. ID Hospital in Gangnam, Wonjin in Gangnam, and Braun in Seocho all participate and will print the refund slip at checkout rather than mail it later. If your clinic is not on the list, ask before you pay the deposit, because switching mid-treatment forfeits the refund on money already wired. For district-level context on where these clinics cluster, our Gangnam vs Apgujeong two-jaw guide covers the practical geography.

HOW THE PAPERWORK FLOWS

At the clinic you receive a Medical Tax Refund slip at the moment of payment, printed with your passport number, the treatment code, the pre-tax amount, and the VAT line. Some clinics apply the refund immediately as a discount on the final invoice, which is called instant refund and is capped at ₩300,000 per transaction. For anything above that cap the clinic prints the slip and you claim at the airport. Keep the slip with your passport, do not fold it through the magnetic strip, and do not pack it in checked luggage.

AT INCHEON ON DEPARTURE

Head to the tax refund desk before you check in your bags if the refund is above ₩75,000, because customs may want to see evidence that the treated person is leaving the country. The desks sit near the departure hall entrances in Terminal 1 and Terminal 2, open roughly 06:00 to 22:00. Cash refunds in KRW process on the spot for amounts under ₩500,000. Larger refunds go to a credit card or wire transfer, which lands 2 to 4 weeks later. If you miss the desk, both Global Tax Free and Global Blue operate mailback envelopes that let you post the slip within three months of departure, but the processing fee is higher.

WATCH FOR THESE THREE TRAPS

First, deposits paid before arrival are a grey zone, and some clinics do not include wire-transfer deposits on the refundable base, so insist on a single consolidated invoice at check-out. Second, the six-month residence rule is strict, and anyone holding a D-type long-stay visa or E-type work visa is excluded regardless of passport. Third, cash discounts offered as a workaround to the VAT line usually mean the clinic is not registered, and you lose the refund entirely. Our breakdown of insurance letters for Seoul plastic surgery and the medical visa rules by country covers the adjacent paperwork most patients forget until the last day.

HOW MUCH YOU CAN REALISTICALLY EXPECT BACK

  • 01Primary rhinoplasty at ₩5,500,000: refund roughly ₩380,000 to ₩440,000
  • 02Full breast augmentation with Motiva implants at ₩9,500,000: refund roughly ₩660,000 to ₩760,000
  • 03Two-jaw surgery at ₩18,000,000: refund roughly ₩1,250,000 to ₩1,440,000
  • 04SMAS facelift with neck lift at ₩22,000,000: refund roughly ₩1,540,000 to ₩1,760,000
  • 05360 liposuction at ₩11,000,000: refund roughly ₩770,000 to ₩880,000
  • 06FUE hair transplant 3,500 grafts at ₩7,000,000: refund roughly ₩490,000 to ₩560,000

Those figures assume a clinic that is registered under the scheme and that bills the full procedure on one invoice. Package deals that bundle anesthesia, hospital stay, and aftercare into one line usually give the cleanest refund. Line-item invoices split across departments can drop the refundable base if the clinic treats the anesthesia line as separately coded.

IS IT WORTH ROUTING SURGERY CHOICE AROUND THE REFUND?

On a ₩20,000,000 budget the difference between a registered and unregistered clinic can be ₩1,400,000, which is the price of a business-class return seat to Southeast Asia or two extra recovery nights in Apgujeong. For procedures above ₩10,000,000 it is worth filtering shortlists to foreigner-designated clinics only. For a ₩3,000,000 non-incisional double eyelid, the refund is small enough that surgeon fit should decide, not VAT. The point of the scheme is to make the headline price less punishing, not to steer you toward the wrong hands.

HOW DOES THE REFUND ACTUALLY GET CALCULATED?

The clinic bills the surgery base plus 10% VAT. The licensed refund operator takes a commission of roughly 2 to 3 percent of the VAT amount for processing, which is why the headline 10% returns as 7 to 9 percent net. Operators publish their fee schedules on their websites, and the clinic usually has the operator-specific slip pre-printed in the billing office.

CAN I CLAIM THE REFUND IF I HAD SURGERY ON A PREVIOUS TRIP?

Only if you are still inside the three-month window from the treatment date and still hold a valid departure within that window. If you flew home already and did not claim at Incheon, the mailback route is your only option, and the slip must be submitted within three months of the treatment date, not the departure date. Clinics will reissue a lost slip if you email the international coordinator within that window.

DOES THE REFUND STACK WITH THE GENERAL TOURIST VAT REFUND?

No. Medical VAT runs on a separate ledger from retail VAT. Shopping bags, Olive Young hauls, and hotel receipts queue at the general tourist desk at the airport, and the medical slip goes to the medical refund operator's counter. Both desks sit in the same tax refund zone at Incheon but they process different forms.

WHAT IF MY CLINIC SAYS THE REFUND IS ALREADY APPLIED?

Ask to see the instant-refund slip anyway, because customs on the way out may still want to verify. Instant refund caps at ₩300,000, so anything beyond that must still go through the airport desk even if the clinic deducted a portion at checkout. The slip will show the deducted amount and the residual, and the airport desk refunds the residual only.

WHICH CLINICS ARE FOREIGNER-DESIGNATED FOR THE SCHEME?

The current Seoul list includes Girin, Grand, ID Hospital, Wonjin, Braun, April31, Dr. Tak, Divine, Light, Answer, Nana, VG, GG, Banobagi, and ME Cosmetic Clinic among others. The MOHW list is updated quarterly and visible on the Korea Health Industry Development Institute site. If a clinic promises the refund but is not on the list, treat it as a red flag and ask for the registration number before paying.
