CHIN IMPLANT VS GENIOPLASTY SEOUL 2026: WHICH CHIN SURGERY?

A technical head-to-head of silicone chin implants and sliding genioplasty in Seoul, with KRW costs, recovery, and the verdict by face type.

Chin surgery in Seoul splits into two very different operations that get thrown into the same consultation. A chin implant adds projection by seating a shaped silicone or Medpor piece against the front of the mandible. A sliding genioplasty cuts the chin bone, repositions the segment forward, backward, up, down, or sideways, and fixes it with titanium plates. The right choice depends on what your chin is actually doing wrong, how much movement you need in which axis, and whether you want something reversible or permanent. Prices in Seoul differ by a factor of two to three between the two, and so does recovery.

WHAT A CHIN IMPLANT ACTUALLY IS

A chin implant is a pre-shaped alloplastic piece, usually smooth silicone or porous polyethylene (Medpor), that a surgeon inserts through either a small cut inside the lower lip or under the chin, then screws or sutures against the periosteum of the mandible. Seoul clinics stock a wide range of pre-formed sizes, typically 3 to 10 millimetres of forward projection, with anatomic shapes that wrap the sides of the chin to blend into the jawline. The operation takes 30 to 60 minutes under IV sedation and goes home the same day. Scar is hidden intraorally or in the submental crease. The implant is removable, which is both its biggest advantage and the reason some surgeons prefer bone work instead.

Silicone is the dominant implant material in Korean practice because it is smooth, easy to position, and extractable in a 15-minute revision if the patient dislikes the result. Medpor is harder, bonds with bone and surrounding tissue over time, resists displacement, but is painful to remove once integrated. A handful of Seoul clinics such as JW Plastic Surgery and Grand Plastic Surgery carry both materials and will make the implant choice based on your skin thickness, soft tissue envelope, and whether you want long-term stability or short-term reversibility. Expect to pay ₩3,000,000 to ₩6,000,000, or around $2,100 to $4,300, including the implant, surgeon fee, anesthesia, and basic aftercare.

WHAT A SLIDING GENIOPLASTY ACTUALLY IS

Genioplasty is bone surgery. The surgeon enters through the inside of the lower lip, exposes the front of the mandible, and uses a reciprocating saw to make a horizontal cut below the roots of the lower teeth. The freed chin segment can then be moved in any of three dimensions. Forward for retrognathia, backward for an over-projected chin, upward for a long lower face, downward to lengthen a short chin, or asymmetric to correct a crooked chin. The segment is held in position with titanium miniplates and self-tapping screws that stay in permanently. Operating time runs 60 to 90 minutes under general anesthesia, with an overnight hospital stay at most clinics.

Because the bone itself moves, genioplasty is the only chin procedure that corrects vertical height, asymmetry, or a receding chin that needs more than six to seven millimetres of forward advancement. It also changes the attachment line of the mentalis muscle, which can improve lower-lip posture and reduce chin dimpling. ID Hospital, VG Plastic Surgery, and Girin Plastic Surgery do high volumes of sliding genioplasty as part of their facial contouring and two-jaw programs. Price in Seoul lands between ₩6,000,000 and ₩12,000,000, roughly $4,300 to $8,600, with hospital-based programs trending toward the top of that band.

PROJECTION, HEIGHT, SYMMETRY: WHICH PROBLEM ARE YOU SOLVING?

An implant adds projection only. If you look at a side profile and the chin sits behind a vertical line dropped from the lower lip by three to six millimetres, an implant can meet the line without touching anything else. Vertical length, width, and asymmetry stay exactly as they were. For a patient whose chin is short in height, or whose chin tilts to one side, or whose chin is both deficient forward and vertically long, an implant will not solve the problem and may make it look worse by stacking projection on top of a long chin.

Genioplasty wins on anything multi-axis. It is the standard answer for a long chin that also recedes, a chin that deviates from midline by two millimetres or more, or a chin that needs more than seven millimetres of advancement, which exceeds what an implant can safely hold without edge visibility. It is also the correct operation when the patient has had orthognathic planning done and the chin needs to follow the mandible to a specific coordinate. Seoul contouring surgeons who do bimaxillary work almost always default to genioplasty for the chin component because it integrates with the jaw plan rather than fighting it.

  • 01Chin implant: projection only, up to 6-7mm, reversible, intra-oral or submental scar, ₩3M to ₩6M, 30 to 60 minutes under sedation.
  • 02Sliding genioplasty: projection plus vertical plus horizontal plus rotational, titanium fixation, permanent, intra-oral scar, ₩6M to ₩12M, 60 to 90 minutes under general anesthesia.
  • 03Combo work: either chin surgery can be paired with jaw-reduction, zygoma reduction, or two-jaw at the same operation, which many Seoul clinics quote as a bundled rate.

RECOVERY, RISKS, AND SCAR

Implant recovery is the shorter one. Swelling peaks at day three, drops sharply by day seven, and most patients are presentable in public without makeup by day ten. The intra-oral stitches dissolve on their own. Soft diet runs five to seven days. Numbness in the lower lip and chin, from stretch on the mental nerve, is common for two to six weeks and almost always resolves. Implant-specific risks include migration, visible edges in a thin soft-tissue envelope, bone resorption under the implant over years, and capsular contracture, though contracture is far less common in the chin than in breast implants.

Genioplasty recovery is longer because bone has to heal. Swelling and bruising peak at day five, significant swelling persists for two to three weeks, and final bone remodeling takes three to six months. Numbness of the lower lip and lower incisors is near-universal in the first month and can take six to twelve months to fully resolve, with a small risk of permanent sensory change if the mental nerve is traumatized during the cut. The titanium plates stay forever and are usually asymptomatic, though a small percentage of patients ask for plate removal at the one-year mark. For international patients, flying home is reasonable at day 10 after an implant and day 14 to 21 after a genioplasty, assuming no drains and no fever.

WHICH CHIN SURGERY IS RIGHT FOR YOUR FACE?

If your chin is simply retruded by three to six millimetres and the rest of the lower face is in balance, a silicone implant is the fastest, cheapest, most reversible answer. Most Seoul consultations for a mild weak chin end with an implant recommendation because the risk-to-reward ratio is good and the result is immediate. The best candidates are patients with adequate soft-tissue thickness, no vertical height issue, no asymmetry, and a tolerance for the small chance of a revision if the size turns out wrong on healing. Pair the consultation with photos from side profile and three-quarter angles, and ask the surgeon to try sizing templates in-office rather than committing to a size over video.

If any of the following apply, book a genioplasty consult instead of an implant consult. A chin that is vertically too long or too short, a chin that deviates from midline, a planned bimaxillary two-jaw surgery, a history of failed implants, a desire for projection beyond seven millimetres, or a thin soft-tissue envelope that would show implant edges. Patients who are already in Seoul for jaw contouring or V-line surgery frequently add a genioplasty at the same operation because the incremental cost is small and the final profile is more harmonious when the chin is designed into the plan rather than bolted on.

HOW MUCH DOES CHIN SURGERY COST IN SEOUL IN 2026?

Silicone chin implants run ₩3,000,000 to ₩6,000,000, or about $2,100 to $4,300. Sliding genioplasty runs ₩6,000,000 to ₩12,000,000, or about $4,300 to $8,600. Hospital-based programs with overnight stay, in-house anesthesiologist, and 3D CT planning sit at the top of each band. Add VAT at 10% if the clinic quotes net prices, and check the VAT tax refund rules because much of the surgical fee qualifies for a foreign-patient refund.

IS CHIN IMPLANT REVERSIBLE AFTER YEARS?

Silicone implants are removable at any time with a short intra-oral revision procedure. The capsule that forms around the implant can be trimmed or left in place, and most patients return to normal appearance within two to three weeks of removal. Medpor is a different story because tissue grows into the porous surface, so removal is longer, more invasive, and may leave more residual change. Sliding genioplasty is not reversible in a practical sense because the bone heals in its new position, though a second osteotomy can move the chin again if indicated.

CAN GENIOPLASTY AND IMPLANTS BE COMBINED?

Rarely, but yes. A small group of patients needs both forward advancement from a sliding osteotomy and additional soft tissue bulk from an implant layered on top, usually when the native chin is very thin bone and the surgeon wants to avoid over-advancing the segment. Most Seoul contouring surgeons avoid this combination because the long-term interface between implant and plated bone is less predictable. If a surgeon suggests the combination, ask specifically why genioplasty alone cannot reach the planned projection and what the plan would be if the implant needs removal in five years.

WHICH SEOUL CLINICS SPECIALIZE IN EACH?

For implants, high-volume generalist clinics with strong aesthetic rhinoplasty programs tend to be the right choice because implant work overlaps with the same soft-tissue finesse. JW Plastic Surgery in Apgujeong, Grand Plastic Surgery, and Braun Plastic Surgery in Seocho all run high implant volumes with international patient teams. For sliding genioplasty, bone-contouring hospitals do the deepest work. ID Hospital has logged more than 40,000 facial-bone cases, VG Plastic Surgery and Girin Plastic Surgery run full facial-contouring programs with 3D CT planning, and Saero Plastic Surgery adds deep-plane facelift expertise if the chin work is part of a broader lower-face plan.

editor's note

Ask every chin consult to show you the specific size and shape of the planned implant, or the exact millimetre vector and axis of the planned genioplasty. If the surgeon cannot answer in millimetres, book a second opinion before you pay a deposit.
