TAX REFUND IN SEOUL FOR AESTHETIC TREATMENTS 2026: VAT RULES EXPLAINED

What foreign patients actually pay in 10% VAT at Seoul aesthetic clinics, which treatments qualify for the Medical Tourism Tax Refund, and how to claim your won back at Incheon.

The price on the Korean clinic menu almost never matches what hits your card, and most of the gap is tax. Korea charges 10% VAT on cosmetic and aesthetic procedures, and a slice of that is refundable at Incheon if you book the right kind of clinic and keep the right pieces of paper. Here is what qualifies, what does not, and how to actually get the won back before your flight boards.

WHY COSMETIC TREATMENTS CARRY 10% VAT IN KOREA

Medical care for treating illness is VAT-exempt in Korea, which is why a GP visit or a surgical procedure to correct a congenital deformity carries no consumption tax. Cosmetic and aesthetic procedures are classified separately under the VAT law amendment that took effect in 2011, and they carry a flat 10% VAT on the service fee. That rule applies to Botox, filler, Ultherapy, Shurink, Rejuran, skin boosters, laser toning, thread lifting, rhinoplasty, double eyelid surgery, breast augmentation, liposuction, two-jaw surgery, and essentially every elective aesthetic procedure a medical tourist would fly for.

Clinic price lists handle this inconsistently. Some clinics quote prices VAT-inclusive, especially the premium Apgujeong and Cheongdam addresses that work with foreign patients regularly. Others quote the pre-tax figure and tack on 10% at the counter, which is where the sticker-shock stories usually come from. The pattern is covered in detail in our hidden costs guide, and the single most important question to ask any Seoul clinic before you commit is whether the quoted KRW figure already includes VAT or not.

The 10% also applies to any add-on service billed by the clinic. If a consultation reveals an upsell such as a second syringe of filler, a bonus HIFU zone, or a package upgrade, that incremental fee carries VAT too. Reading our how to read a Korean clinic price menu guide first will save you from assuming the discount price on a Naver booking screenshot is the final number.

THE MEDICAL TOURISM TAX REFUND SCHEME, IN PLAIN TERMS

Korea runs a dedicated refund scheme for foreign medical tourists, separate from the general shopping tax refund tourists use at department stores. It is formally called the Medical Tourism Tax Refund, and it was expanded in 2021 to cover aesthetic and dermatological procedures that previously fell outside the rules. The refund is operated by two licensed providers, Global Tax Free and KT Tax Refund, and most participating clinics are integrated with one of them. The provider logo is usually posted at reception or appears as a line item on the receipt.

The refund works like this. The clinic logs your procedure and your passport into the refund system at the time of payment, prints a Medical Tax Refund slip, and gives you back a stub for the airport. You then present the stub, your passport, your boarding pass, and ideally proof of the treatment at the dedicated medical refund desk at Incheon or Gimpo before departure, and the eligible VAT amount is credited back in won cash, to your card, or to Alipay. The entire process is designed for departures within 90 days of treatment, which matches the typical medical tourism visa window.

The refund is not the full 10%. After the provider's service fee and the sliding tier structure, most patients receive roughly 5% to 7% of the pre-tax procedure value back. On a ₩3,000,000 rhinoplasty that is still ₩180,000 to ₩210,000, which covers an airport hotel night and the Incheon transfer. On a ₩20,000,000 facelift it is a meaningful ₩1,200,000 to ₩1,400,000 refund, enough to fund the whole trip. The exact tier is published annually by the Ministry of Economy and Finance.

WHO QUALIFIES AND WHO DOES NOT

The refund is restricted to foreign nationals who have been in Korea for less than six months at the time of treatment and who depart within three months of the procedure. Korean citizens, long-term resident-visa holders, and anyone employed in Korea are excluded. Diplomatic passports and crew passports are also excluded under the aviation and consular rules.

Procedures must be performed at a clinic registered with the Korea Health Industry Development Institute as a designated medical institution for foreign patients. Not every Seoul clinic is registered, and that is the single biggest gating factor. The headline Apgujeong, Cheongdam, and Gangnam clinics that advertise in English are almost always registered, as are the larger hospital groups and the clinic chains that run English Kakao channels. Smaller Korean-domestic clinics tucked into Sinnonhyeon or Sinsa often are not, which does not mean they are worse, only that your VAT does not come back.

  • 01Procedures that qualify: Botox, filler, Ultherapy, Shurink Universe, Thermage, Rejuran, skin boosters, laser toning, chemical peels, RF microneedling, exosomes, PRP, mesotherapy, rhinoplasty, double eyelid surgery, blepharoplasty, breast augmentation, liposuction, facelift, two-jaw surgery, zygoma reduction.
  • 02Procedures that do NOT qualify: anything billed as corrective or reconstructive under medical necessity (because it is VAT-exempt in the first place), dental implants and orthodontic work (handled under a separate dental scheme), and any treatment where the invoice is issued to a company rather than the patient personally.
  • 03Payment methods that work for refund: Korean credit card, international Visa/Mastercard/Amex, Alipay, and cash. The refund method mirrors the payment method where possible.

HOW TO ACTUALLY CLAIM THE REFUND AT INCHEON

The claim process has three touchpoints. At the clinic, confirm the treatment is logged into the Global Tax Free or KT Tax Refund system before you leave the reception desk, and collect the printed Medical Tax Refund slip along with the standard tax invoice. Keep the slip with your passport, not in checked luggage. If the clinic staff look blank when you ask for the refund slip, the clinic is not enrolled, and no amount of paperwork at the airport will fix that after the fact.

At your hotel the night before departure, scan or photograph every receipt and refund slip as a backup. Combine them in a single folder. If you also did shopping that qualifies for the regular 10% tourist refund, keep the two stacks separate because they are processed at different desks.

At Incheon, the Medical Tax Refund counters sit on the third floor of Terminal 1 near Gate D, and in Terminal 2 near Gate 2 at the departures level. Both terminals also have self-service kiosks that handle smaller refunds without a staff queue, which usually clear in under five minutes. Go to the counter before you check your luggage if any refund slip is tagged for inspection, because the officer may ask to see post-op markers, dressings, or implant certificates. The transfer route from the airport to central Seoul is covered in our Incheon transfer guide, and reverse-timing the airport leg is the same, just add 45 minutes for the refund desk during peak departure windows.

Refunds in cash are paid in won at the counter. Card refunds post back to the original payment card in 7 to 60 days depending on the issuing bank. Alipay refunds are usually instant. For any amount over ₩75,000 you must have the slip stamped by customs before clearing security, so budget 20 minutes extra if your treatment total exceeded about ₩1,500,000.

THE MISTAKES THAT KILL THE REFUND

The three failures I see repeatedly: paying in cash at a non-enrolled clinic, booking through a third-party agent who invoices in their own name, and leaving within 24 hours of a procedure that still has visible dressings. Cash paid at a non-enrolled clinic generates no VAT paper trail at all, so there is nothing to claim. Agent invoicing routes the receipt through a Korean company, which disqualifies it. And some officers will refuse to stamp a refund slip for a high-value procedure if the patient cannot show the surgical site at inspection, which only matters when the treatment finished less than a day before the flight.

The fix for all three is sequence. Book the clinic directly, confirm Medical Tax Refund enrollment at consultation, pay in your own name on your own card, and schedule flights at least 48 hours after the procedure. Our flying home after Seoul plastic surgery guide handles the recovery-timing side of that in detail. Treat the refund as a budget line to protect, not a bonus to hope for.

WHAT THE REFUND ACTUALLY CHANGES ABOUT TRIP BUDGETS

On a ₩500,000 Botox and skin booster run, the refund is roughly ₩25,000, which is a nice tip-of-the-cab win and nothing more. On a ₩4,000,000 injectables stack across three visits, you are looking at ₩200,000 to ₩280,000 back, roughly a long-stay night at a serviced apartment near Gangnam. On a surgical package in the ₩15,000,000 to ₩30,000,000 range, the refund genuinely funds the recovery leg of the trip.

That is why aesthetics-only tourists tend to ignore the refund and surgery tourists do not. If your trip is a weekend of injectables, the refund matters less than picking a credible clinic. If your trip is a facelift or two-jaw, the refund is a four-figure variable that should be in the spreadsheet from the first consultation.

  • 01₩500k Botox + boosters trip: refund ≈ ₩25,000 (covers one airport coffee).
  • 02₩4M injectables stack: refund ≈ ₩240,000 (covers a serviced apartment night).
  • 03₩15M rhinoplasty + facial contouring: refund ≈ ₩900,000 (covers Incheon transfers both ways plus two recovery nights).
  • 04₩30M two-jaw + facelift combo: refund ≈ ₩1,800,000 (covers a full week of recovery accommodation).
